For first time investors in the Sun Life Prosperity Funds


For new investors, the Investor Profile Questionnaire is required to assess their risk profile and to determine the Sun Life Prosperity Fund suitable for their investment objectives.

For new investors, the Investor Information Form must be fully accomplished and submitted with the Investor Profile Questionnaire.

For new investors opening a an In Trust For Account, the Confirmation of ITF Agreement Form must be fully accomplished and submitted. 

The Order Ticket for Initial Investment must be submitted with the Investor Profile Questionnaire and the Client Information Form upon initial account opening.

While the required forms for opening a Sun Life Prosperity Fund account are available, we highly encourage you to get in touch with a Sun Life Mutual Fund Advisor to thoroughly understand the benefits and risks in investing in mutual funds. SLAMC shall only accept account opening forms duly signed by a Sun Life Mutual Fund Advisor.

For new investors who wish to enroll in the Sun Synergy program, the Sun Synergy Client Authorization Form must be submitted. For more information on the Sun Synergy program, you may download the attached brochure.

For current investors in the Sun Life Prosperity Funds


For current investors wishing to add to their existing investments. A validated deposit slip or confirmation of money transfer must be submitted with the Order Ticket for Subsequent Investment

For current investors who would like to enroll in the Sun Life Prosperity Funds' Auto-Invest Program, the following forms must be submitted:

  1. SLAMCI Auto-Invest Enrollment Form
  2. SLAMCI Auto-Invest Cancellation Form
  3. SLAMCI Auto-Invest Amendment Form 
  4. BDO ADA Form
  5. BPI ADA Form
  6. Security Bank ADA Form
  7. Metrobank ADA Form

For current investors wishing to redeem from or transfer their existing investments. The original share certificate (if issued) of the investment being redeemed for withdrawal or transfer must be submitted with the Order Ticket for Redemption/Interfund Transfer, and the Request for Check Uncrossing Form (applicable for requests for uncrossed checks).

For current investors wishing to update their personal account information (i.e. Change of Name, Change of Legal Residence, Change of Citizenship, etc.) and enroll into MF Services a Enrollment to MF Services a Non-Financial Amendment Form must be submitted.

For current investors who wish to enroll in the Sun Synergy program, the Sun Synergy Client Authorization Form must be submitted. For more information on the Sun Synergy program, you may download the attached brochure.

Completed forms may be submitted to your Sun Life Mutual Fund Advisor or at any of the Sun Life Financial offices.
